Fault
Also: fault cause
A defect or abnormal condition that can lead to an incorrect internal state.
Faults may be transient, intermittent, permanent, design, manufacturing, environmental, or operational and require defined detection scope.
Example A loose encoder connector is a fault that may freeze counts.
Common misconception A fault is the cause, not necessarily the externally observed failure.
